BlazeVIEW Extend Student Access Request

Purpose: Faculty may use this form to request extension of access to a BlazeVIEW course section for the purpose of allowing a student to complete work for resolution of an Incomplete grade.

NOTE: Students normally have access to BlazeVIEW sections for a period of 30 days after the official end date for the section. Upon faculty request, student access (for a specific student) will be extended for a period of one semester.

Required Fields (marked in red):Instructor Name, Instructor E-mail Address, Section, Student's Full Name, Student's VSU Email, Security Image Verification.
