REQUEST PROGRAM CHANGE

This form is for CURRENT MSW students to request departmental consideration for a change of program (slowing down, changing cohorts, leave of absence). For additional assistance, call 229-249-4864 or email socialwork@valdosta.edu. Please read the following notes carefully:

1) Meet with your ACADEMIC ADVISOR first about your options and then use this form as an official request for departmental consideration.

2) If you are requesting WITHDRAWAL or a LEAVE of ABSENCE from the program, be aware that you may be required to re-apply to the program if you decide you want to re-enroll.

3) If you are requesting a CHANGE OF COHORT, be aware that changes into the web program are approved only in extreme circumstances. Strict class size limits prevent students from switching between groups. Academic progress and program performance will also be considered.
