Form Purpose: To report a problem connecting to an electronic resource, please fill in this form as completely and accurately as possible.

IMMEDIATE HELPFUL HINTS: Try logging into the Anywhere Library Access with your BlazeVIEW account and then access the problem electronic resource or read the Technical Problems FAQ.  When in GALILEO, users are encouraged to directly report access problems via the the "Contact Us" link towards the top of the screen.  Forgot your BlazeView password?  Use the Self-Service Password Reset utility.
